Community Health Program Associate

Position title:    Community Health Program Associate
Classification:   Non-exempt, Part-Time (20 hours per week)

Position summary

The Community Health Program Associateis part of a team that seeks to address health disparities on the South Coast. This position is the primary point of contact for high-needs individuals and families who are experiencing complicated health or medical situations and/or enrolled in numerous Puente economic security, behavioral health, and physical health programs. The Community Health Program Associatealso works closely with the entire Community Health team to help coordinate programs and services; provide case management; and maintain data.

Essential Job Responsibilities

The Community Health Program Associate’s responsibilities include the following functions:

Case Management

  • Coordinate multiple aspects of care for participants, including specialty care, translation, scheduling appointments, and patient accompaniment when necessary
  • Assess participants’ holistic medical and non-medical needs in order to create a proactive care plan that empowers the participant to maintain or regain control of their care
  • Ensure each participant has an advocate for needed health services and any needed health education to manage or improve their medical conditions

Program Coordination/Engagement

  • Clinic coordination with mobile dental provider (scheduling with provider, confirmation calls with patients, coordinating with Community Health Workers as necessary)
  • Coordinate referrals to other health providers such as the San Mateo County Field Medicine Team, County Public Nurse, and other Puente teams
  • Assist in coordinating community health events, such as flu shots campaign, field medicine screenings, and Annual Health Fair
  • Promote engagement and participation in Puente’s activities, conducting surveys, keeping records, and assisting with ongoing assessment of community needs
  • Data entry


  • Bachelor’s degree in social work, public health or related field or equivalent combination of education and experience
  • Demonstrated proficiency in translating English to Spanish and Spanish to English
  • Experience in case management preferred
  • Ability to organize tasks and people in an efficient manner. Must possess excellent verbal and written communication skills
  • Excellent skills with Microsoft Office products, including: Word, Excel, Outlook, and PowerPoint
  • Experience with database systems
  • Current California Driver’s License and ability to drive to work-related functions in and around Pescadero, La Honda, Loma Mar and San Gregorio/Southern San Mateo County area

  • Commitment to Puente’s vision of supporting a healthy, inclusive, and sustainable South Coast community
  • Extensive experience with identifying and coordinating critical participant needs and leveraging resources to create sustainable programs
  • Cultural competency to work with residents from diverse backgrounds across boundaries of class, race, and inequality issues
  • Knowledge of the South Coast or experience working in a rural community, preferred
  • Available to work evenings and weekends, as needed
  • Ability to work independently and manage multiple priorities and projects outlined in various work plans
