Position title:        Community Medical Navigator

Classification:       Non-exempt, Full time

Reports to:             Community Health Manager

Position summary

The Community Medical Navigator is part of a team that seeks to address health disparities on the South Coast by creating a medical home for its residents. This position is the primary point of contact for high-needs individuals and families who are experiencing complicated health or medical situations and/or enrolled in numerous Puente economic security, behavioral health, and physical health programs.

Essential Job Responsibilities

The Community Medical Navigator’s responsibilities include the following functions:

  • Assess the participant’s holistic medical and non-medical needs in order to create a proactive care plan that empowers the participant to maintain or regain control of their care
  • Ensure the participant has an advocate for needed health services and any needed health education to manage or improve their medical conditions
  • Coordinate multiple aspects of care to ensure the participant progresses, including specialty care, translation, and patient accompaniment when necessary
  • Transition the participant to the appropriate level of care needed, making, coordinating, and tracking referrals
  • Assess and address psychosocial needs including individual, familial, environmental, etc.
  • Make appointments, help participants navigate healthcare systems, and actively make or manage referrals for those eligible to receive existing or new health-related services, including emergency food, emergency housing assistance, health insurance and CalFresh enrollments, immigration services, translations, phone calls, assistance with obtaining transportation and referrals to other resources
  • Promote engagement and participation in Puente’s activities, conducting surveys, keeping records, and assisting with ongoing assessment of community needs
  • Represent Puente at meetings with external partners and government agencies
  • Perform related duties as assigned


  • Bachelor’s degree in social work, human services, counseling or related field or equivalent combination of education and experience
  • Experience in case management, providing direct services to program participants
  • Demonstrated proficiency in translating English to Spanish and Spanish to English
  • Ability to organize tasks and people in an efficient manner. Must possess excellent verbal and written communication skills
  • Excellent skills with Microsoft Office products, including: Word, Excel, Outlook, and PowerPoint
  • Experience with web-based systems, One-e-App and Covered California portal, a plus
  • Experience with database systems
  • Current California Driver’s License and ability to drive to work related functions in and around Pescadero, La Honda, Loma Mar and San Gregorio/Southern San Mateo County area

  • Works collaboratively with colleagues, institutions, and community members
  • Extensive experience with identifying and coordinating critical participant needs and leveraging resources to create sustainable programs
  • Ability to communicate fluently in English and Spanish, both orally and written
  • Excellent computer skills, including ability to utilize data management software
  • Cultural competency to work with residents from diverse backgrounds across boundaries of class, race, and inequality issues
  • Demonstrated cultural sensitivity and commitment to advocacy for cultural diversity and equity
  • Knowledge of the South Coast or experience working in a rural community, preferred
  • Available to work evenings and weekends, as needed
  • Ability to work independently and manage multiple priorities and projects outlined in various work plans
  • Able to multitask and work on numerous projects at one time
  • Adapt easily to new protocols and changing environments
  • Knowledge of the South Coast and/or have worked in a similar community
